Applicant's signature certifies that he or she holds a valid state resale tax id. He or she will resell the merchandise and that he or she will be purchasing from PDU CAT under this resale certificate in the form of tangible personal property in the regular course of his or her business operations, and he or she will do so prior to making any use of the item(s) other than demonstration and display while holding the item(s) for sale in the regular course of his or her business. He or she understands that if he or she uses the item(s) purchase under this certificate in a manner other than as just described, he or she will owe use tax based on each item's purchase price or as otherwise provided by law.
Applicant's signature attests accuracy of above information, financial responsibility, ability and willingness to pay our invoices in accordance with our terms. Applicant agrees to pay reasonable attorney fees plus interest in cases of default in payment in compliance with terms. Applicant agrees that all sales shall be deemed to have been made, and jurisdiction and venue for any legal action instituted for collection, thereupon, shall be vested in the county of Shelby, state of Tennessee, in the appropriate courts thereof. Applicant waives personal service, and consents that service of process shall be made by certified mail, return receipt requested, addressed to the above billing address.
